Black Box Warning: Methylene Blue

WARNING: RISK OF SEROTONIN SYNDROME WITH CONCOMITANT USE OF SEROTONERGIC DRUGS

Methylene blue may cause serotonin syndrome, a potentially life-threatening condition, when administered with serotonergic drugs such as SSRIs, SNRIs, MAOIs, or tricyclic antidepressants.

Mechanism: Methylene blue inhibits monoamine oxidase A (MAO-A), an enzyme responsible for serotonin breakdown. In individuals taking serotonergic medications, this inhibition may lead to excessive serotonin levels.

Symptoms of Serotonin Syndrome:

  • Agitation, restlessness, or confusion
  • Rapid heart rate or high blood pressure
  • Muscle rigidity, loss of coordination, or twitching
  • Heavy sweating, nausea, or vomiting
  • Severe cases: high fever, seizures, or loss of consciousness

Contraindications: Methylene blue is contraindicated in patients using serotonergic drugs unless under emergency conditions with strict monitoring. Serotonergic medications should be discontinued at least two weeks before methylene blue administration whenever possible.

Side Effects:

  • Common side effects: headache, blue/green discoloration of urine, diarrhea, nausea, and muscle twitching (myoclonus).
  • Severe side effects: hemolytic anemia (particularly in individuals with G6PD deficiency) and interference with medical tests or monitoring devices.

Precautions and Storage:

  • Store in a cool, dry place away from sunlight. Avoid freezing.
  • Patients on serotonergic drugs must be closely monitored.
  • Avoid excessive light exposure as methylene blue is photosensitizing.
